All primary practice disclosures — including the Notice of Privacy Practices, HIPAA telehealth consent forms, Good Faith Estimates, practice policies, consent forms, and accessibility statements — are managed and signed through the Headway provider onboarding portal during your initial registration.
This website hosts no contact forms, intake questionnaires, or input fields of any kind. There is nowhere on this site to submit your name, symptoms, or contact details, and none are collected.
All registration data, eligibility tracking, and insurance verification occur externally on Headway’s secure, HIPAA-compliant infrastructure. That boundary is deliberate.
Telehealth practice is governed by where the client is physically located during the live session, not where the clinician is based. Your location is confirmed at the start of every appointment.
